  • Disclosure regarding Information Disclosed for Bankruptcy Assistance

  • New Client Sheet:
    fill this out and bring with you to the initial appointment to speed up the appointment.  Be sure to list names and amounts of all creditors to whom you owe money.  Do not mail, email, or fax to us.  We need to meet with you in person to discuss your case.

    Budget:
    While I usually do not require this at the initial appointment, you may wish to fill this out before you come in to assist in advising you between chapter 7 and chapter 13.

    Collier TopForm™ Web-Based Questionnaire:
    For our existing clients who wish to fill out the Bankruptcy questionary online and submit it to our office. You will need to call our office to obtain the attorney ID number. This service is only available for access by clients who have retained our office.

    Statement of Clients Rights:  Required rights and responsibilities for representation in a bankruptcy case per a proposal presented to the Honorable Michael Williamson, United States Bankruptcy Judge, Middle District of Florida.  While this form would normally apply only in cases in his court, and only after the formal ruling in the case in which this is pending, this firm abides by the rights and responsibilities in all bankruptcy cases.  (Updated copy of statement being obtained).
